Parent's Responsibilities

Every childcare record will be maintained in a confidential manner. All active records shall be kept in the childcare center at all times and shall be available to the Department of Health & DCFS for review. Records will be current, complete, and legible. (See Appendix C of items to be included in the personnel files).

Records on children and/or employees no longer in the childcare center will be placed in an inactive file. The file will be maintained by the childcare center for a period of five years. Each day care center is responsible for submitting the monthly report.

The following rules and regulations govern the conduct of all childcare centers.

A) The center will maintain written policies that are reviewed and updated annually. These written policies must be available for review.

a. Transportation
b. Drop off children-not at the curb but in classroom signing them in/out
c. Release of children-name must be on file.
3. Emergency evacuation plan, drill procedures and schedule
4. Emergency medical treatment, including procedures to following the event of poisoning.
5. Notification of parents for emergencies
6. Discipline policy
7. Parent participation
8. Sanitation plan/Universal precaution
a. Diapering/toileting (if applicable)
b. Hand washing
c. Spilled waste (body fluids; vomits, blood, urine, sputum, stool)
d. Pets and animals (health certificate)
10. Medication policy
11. Facilities and equipment maintenance plan (indoor and outdoor)
B) There shall be a policy, which states that childcare centers must report exposure to contagious or communicable diseases to the parents, public health authorities and staff.
C) There shall be a policy, which states that the use of tobacco, in any form is prohibited in the childcare center There shall be a policy, which states that the Abused and Neglected Child must report evidence of child abuse to authorities as required.

A) Applicant must be two years old, potty trained (with exception, see potty training policy). Current immunization, with TB and leas test results is required. Must provide certified physical exam verified by a physician). Physical must be upgraded annually for all children attending daycare center.
B) Copy of original certificate of birth must be provided to the center staff for duplication.
C) All children must attend school daily, except in a case of illness, emergencies, or parent’s vacation
1. The center is open from 6:00 am to 9:00 pm. Children should be in school by 9:30 am. Students arriving after 9:30 am will not be admitted.
2. Students must be picked up by 9:00 pm.
3. Children should not come to school with:
a) Fever 100 or above
b) Infectious diseases
c) Special condition that a health consultant has advised puts him/her at risk
d) Vomiting due to upset stomach
e) Diarrhea
f) Constant runny nose not caused by allergies
g) Pain
4. On return to school, after a contagious disease, parents must submit a doctor’s statement.
D) Open Arms does not provide transportation to and from the center
E) All parents are required to accompany their child into and out of the daycare center daily. Parents MUST sign them in and out daily. It is responsibility of the parents to ensure their child’s safety during pickup and drop-off times. For the safety of the child(ren), parents are not allowed to drop off child(ren) at the door.
F) Children are to be released to parent or alternate adult whose names are listed in the child’s file only. Valid photo identification is required, no exception.
G) Teacher, assistant, or administrative staff members are required to supervise all playground activities at all times. Children must be picked up no later than 9:00 PM. According to State laws, your child may not be released to any person other than yourself or someone whom you have authorized on your enrollment form. You may at any time update/expand this form for emergency pick-up. The person you designate to pick-up your child must show identification and give it to an administrative staff member to make a copy for your child’s file. At the discretion of the Director, at 9:30pm, if the Director or other authorized personnel has not spoken to an adult who is in route to the center, 311/911 will be called and the Police Department will be notified of the incident. We will attempt to have the child picked up by the Police. If the estimated wait time is more than 30 minutes, we will contact the Department of Children and Family Services and notify them that no one has not been picked up.
